Selpercatinib Improves EFS in RET Fusion-Positive NSCLC.

Summary

Results from the LIBRETTO-432 study indicate that adjuvant selpercatinib substantially improves event-free survival in patients with early-stage RET fusion-positive non-small cell lung cancer (NSCLC). This finding establishes selpercatinib as a new standard of care for this rare disease. Nevertheless, challenges persist regarding patient identification and access to RET fusion screening.

Analysis

From a clinical perspective, the LIBRETTO-432 study, while not explicitly stated as a Phase III trial in the abstract, presents robust results sufficient to establish a new standard of care, indicating a high level of evidence. Selpercatinib acts as a targeted therapy for patients with RET fusions, making the RET fusion a predictive biomarker for response. The substantial improvement in event-free survival warrants an immediate change in clinical practice for this patient population. The main challenge now lies in implementing systematic RET fusion screening to identify eligible patients, which could take 1-3 years for full integration.
