Serum-based BRAF V600E detection improves early diagnosis of papillary thyroid carcinoma: A prospective diagnostic accuracy study compared with ultrasound-guided cytology.

Summary

This prospective study evaluated the clinical value of serum-based BRAF V600E mutation detection, alone and in combination with ultrasound-guided fine-needle aspiration cytology (US-FNAC), for the preoperative diagnosis of papillary thyroid carcinoma (PTC). The study included 145 patients with small thyroid nodules. Results indicated that serum BRAF V600E detection, while comparable to US-FNAC in overall performance, showed significantly higher sensitivity for papillary thyroid microcarcinoma (micro-PTC). The integration of both methods substantially improved overall diagnostic performance, suggesting a complementary approach for earlier diagnosis and better risk stratification.

Analysis

🔴 CLINIQUE : This prospective phase II diagnostic study evaluates the addition of a serum biomarker to standard diagnosis. The combination of serum BRAF V600E detection and US-FNAC significantly improved diagnostic performance (sensitivity 81.25%, specificity 85.19%, AUC 0.873) compared to either method alone. This biomarker is particularly promising for the diagnosis of papillary thyroid microcarcinoma, where its sensitivity (94.12%) surpassed that of US-FNAC (64.71%). These findings could justify larger clinical trials and potentially alter clinical practice by offering a less invasive approach for early diagnosis and risk stratification, with a potential clinical impact within 3-5 years. 🟢 BIOMOL : BRAF V600E mutation detection was performed using allele-specific quantitative PCR on serum samples, a robust and sensitive technique. The use of a liquid biopsy (serum) is a major advantage, offering a less invasive approach than US-FNAC. BRAF V600E acts here as a diagnostic biomarker, particularly effective for micro-PTC. Although sensitivity was limited for poorly differentiated PTC, its high performance for micro-PTC suggests significant translational value for screening and early diagnosis in specific patient populations.
