Efficacy and prognostic factors of first-line bevacizumab plus chemotherapy in elderly patients with advanced driver gene-negative lung adenocarcinoma: A retrospective study.

Summary

This retrospective study evaluated the efficacy and prognostic factors of first-line bevacizumab plus chemotherapy in elderly patients (≥65 years) with advanced lung adenocarcinoma lacking driver gene alterations. Among 169 included patients, the bevacizumab-treated group demonstrated a median overall survival of 47 months and a median progression-free survival of 24 months. Multivariate analysis identified bevacizumab as an independent favorable prognostic factor for both progression-free and overall survival. Biomarkers related to inflammation, nutrition, and tumor burden were also associated with prognosis.

Analysis

This retrospective study provides clinical data on the efficacy of first-line bevacizumab in elderly patients with advanced driver gene-negative lung adenocarcinoma. Although the level of evidence is limited by the retrospective nature of the study, the median overall survival (47 months) and progression-free survival (24 months) observed in the bevacizumab group are promising. The identified biomarkers (PNI, VEGF-A, CEA, CA-125, NWR, NLR) are prognostic and could aid in risk stratification and patient selection. These findings warrant prospective studies to confirm these observations and potentially integrate these biomarkers into clinical practice to optimize treatment strategies for this specific population, with a potential clinical impact within 3-5 years.
